APPLICANT ACKNOWLEDGMENT OF COMPANY DRUG TESTING

As a condition of employment with this Company, I understand that, in accordance with FAA Anti-Drug Testing Program, Department of Transportation (DOT) Regulations, and Air Serv Corporation Company Policy, I will be required to take a pre-employment drug test. The Anti-Drug Program and DOT Regulations require urine and/or saliva testing for the following five specific drugs – marijuana, cocaine, opiates, amphetamines, and PCP. If hired, I further understand that I will be part of the Company’s ongoing Drug/Alcohol Abuse testing program which includes random, reasonable suspicion, post accident, return to duty and follow-up testing.

If I either refuse to cooperate with the mandatory drug/alcohol testing program as implemented by Air Serv Corporation, or if I have a verified positive drug test reported to the Company after the careful review of the Medical Review Officer, I understand that I will not be considered for employment.

JURY TRIAL WAIVER AGREEMENT

The Company hopes that, if you become employed, your experience will be a positive one. We also firmly believe that our internal complaint resolution procedures should be sufficient to resolve any problems that may arise between you and the Company. However, we recognize that sometimes, despite everyone’s best efforts, a matter cannot be resolved internally. In those rare instances, we believe that judges are in the best position to fairly resolve these disputes.

This document is a Jury Trial Waiver Agreement (“Agreement”). In exchange for the Company’s consideration of your application for employment and your actual employment (if hired), you are being asked to sign this Agreement as a condition of being employed by the Company. Because we believe that judges are best suited to resolve disputes between you and Company, if you choose to sign this Agreement, the Company also will waive its right to request or demand a jury trial with respect to any lawsuit between you and the Company.

This Agreement does not prevent you from filing charges with administrative agencies such as the Equal Employment Opportunity Commission or the National Labor Relations Board or equivalent state agencies. This Agreement does not take away any of your rights to sue or to seek all of the types of remedies the law allows. It simply provides that a judge will decide any differences that cannot be resolved through the Company’s internal complaint resolution procedures. By signing this Agreement, you waive your right to request or demand a jury trial with respect to any claims under federal or state law (to the extent permitted by the law of the state in which you may be employed) that you may assert against Air Serv Corporation, its affiliates, subsidiaries, divisions, successors, assigns, purchasers, and/or its current, former, and future employees, shareholders, officers, directors and agents (“the Company”). This includes, but is not limited to, claims relating in any manner to your application for employment, your employment (and any of its terms or conditions), or your separation from employment with the Company.

You may choose to reject this Agreement. If you choose not to sign this Agreement, you will not be hired (or, if hired, you will not remain employed) by the Company. If you decide not to sign this Agreement today, you may take it with you and you may consult with any attorney of your choosing to discuss this Agreement before signing it. You may take as much time as you want to consider this Agreement, but you will not be considered for an open position until you have submitted a complete application (including this Agreement).

This Agreement constitutes the entire agreement and understanding between you and the Company and supersedes any prior agreements and understandings, whether written or oral, related to its subject matter. This Agreement cannot be revoked or modified except by a written agreement, signed by you and the General Counsel of the Company. Nothing in this Agreement shall be construed to create an offer of employment or a contract for a definite term of employment, express or implied, or to alter any at-will employment relationship.

IMPORTANT: BY SIGNING BELOW, I AGREE THAT I HAVE HAD AS MUCH TIME AS I WANTED TO CONSIDER THIS AGREEMENT AND THAT I UNDERSTAND I AM GIVING UP AND WAIVING MY RIGHT TO A JURY TRIAL KNOWINGLY, INTELLIGENTLY, VOLUNTARILY, AND FREE FROM DURESS OR COERCION.

It collects personal, educational, and professional information to assess the suitability of candidates. Completing the form accurately is essential for consideration for employment.

  2. Is it necessary to complete all sections of the application form?

Yes, providing complete information on the application form is crucial. Failure to do so may result in the denial of the application. If additional space is needed, attach separate sheets clearly labeled with the relevant section. Ensure all required attachments are present when submitting the application.

The form should be filled out using black or dark blue ink only. This ensures clarity and legibility of the information provided.

Common mistakes

Filling out the Airsev Application For Employment form meticulously is crucial for candidates seeking employment. However, individuals often make errors that may hinder their application process. Here are five common mistakes to be mindful of:

  1. Not providing complete information: It is essential to fill out every part of the application. Omissions can lead to delays or outright denial.
  2. Utilizing incorrect ink: The form specifies the use of black or dark blue ink only. Using other colors can make the document hard to read or scan, complicating the reviewing process.
  3. Failing to attach required additional documents: When more space is needed, candidates should attach separate sheets clearly labeled with the corresponding section of the application. Neglecting this detail can leave an application incomplete.
  4. Offering false information: Honesty on the application is critical. Providing false information is not only grounds for disqualification but can also have legal repercussions.
  5. Overlooking employment history details: The ten-year employment history is crucial, including all gaps. Failure to accurately report this information, or not providing verification for employment gaps, can raise red flags during the screening process.

In summary, applicants should meticulously review their applications for completeness, adhere to specified instructions regarding ink color, attach all required documents, ensure the accuracy of the information provided, and comprehensively detail their employment history. Avoiding these common mistakes can greatly improve the chances of a successful application.

Documents used along the form

When individuals apply for a job with Airsev through the Application for Employment form, a comprehensive approach is often needed to ensure that the application package is thorough and presents the applicant in the best possible light. To support their application, applicants may need to prepare and submit additional documents and forms that complement the information provided in the initial application. Below are five key documents that are frequently used alongside the Airsev Application for Employment form.

  • Resume/Curriculum Vitae (CV): This document offers a detailed overview of the applicant's education, work history, skills, and significant accomplishments. It allows the applicant to elaborate on their qualifications and experiences that make them a suitable candidate for the position.
  • Cover Letter: Serving as an introduction, the cover letter provides applicants with the opportunity to connect their background to the job requirements personally. It explains why they are interested in the position and why they would be a perfect fit for the organization.
  • References List: A document containing contact information for professional references can be crucial. It should include the names, job titles, companies, and contact details of people who can vouch for the applicant's abilities and work ethic.
  • Proof of Eligibility to Work: Applicants may need to provide documents such as a passport, birth certificate, or employment authorization document that verify their identity and eligibility to work in the United States as per legal requirements.
  • Educational Certificates and Transcripts: For positions requiring specific educational qualifications, applicants should be prepared to submit copies of diplomas, degrees, or transcripts that authenticate their educational background.

Including these documents with the Airsev Application for Employment form creates a comprehensive application package that can help employers gain a fuller understanding of the candidate. Each document plays a distinct role in showcasing the applicant's qualifications, professional history, and readiness for the position being applied for, ensuring the selection process is informed and comprehensive.

Similar forms

The Airsev Application for Employment form bears similarity to a standard job application found across various industries. This similarity is clear in its request for general personal information, educational background, previous employment details, and the applicant's availability to start work. Standard job applications serve to gather preliminary information from candidates to determine their fit for a role. They often include sections for the applicant to list their skills, experiences, and qualifications that make them suitable for the position, identical to the content and structure seen in the Airsev form.

Another document similar to the Airsev Application for Employment form is the Consent to Background Check form which is commonly used by employers to conduct a comprehensive review of a candidate’s history. The latter section of the Airsev application where applicants authorize the investigation of their background aligns with this purpose. Both types of documents require the applicant's signature to indicate consent for the employer to perform background, criminal, or credit checks, further ensuring the applicant's fitness and reliability for employment.

The Driver’s License Application is also akin to the Airsev Application for Employment form, especially in sections that request motor vehicle information and driving experience. Just as a Driver’s License Application requires detailed driving history, state of issuance, and any accidents or violations, the Airsev form collects similar data to assess the applicant’s suitability for roles requiring driving. Both documents serve as a means to verify the applicant's capability and legal standing to operate a vehicle, which is critical for positions involving transportation or delivery services.

Employment Eligibility Verification Form (I-9) shares similarities with the Airsev application regarding the requirement for candidates to confirm their legal right to work in the United States. In both documents, applicants must attest to their citizenship or provide proof of eligibility to work, a fundamental step in the hiring process. While the I-9 is a separate legal document necessary for compliance with the Immigration Reform and Control Act, the Airsev application integrates this step into their process, ensuring candidates are aware of the necessity to verify their employment eligibility.

The DOT Applicant Questionnaire, specifically for positions regulated by the Department of Transportation, is closely related to the sections of the Airsev Application for Employment form that focus on DOT compliance. This includes questions regarding previous drug and alcohol testing, which are critical for roles that require adherence to DOT regulations. Both the questionnaire and the Airsev application form aim to identify any past violations or issues that could disqualify the applicant from being considered for positions that have strict safety and regulatory requirements.

Dos and Don'ts

When you're filling out the Airsev Application For Employment form, it’s crucial to ensure that you do it correctly to increase your chances of success. Here’s a helpful guide to steer you right.

Things you should do:

  • Fill out the form using black or dark blue ink: Stick to these ink colors to maintain the form's readability and professionalism.
  • Provide complete information: Incomplete applications can delay the process. If you need more space, attach additional sheets clearly labeled with your information.
  • Attach all required documents: Confirm you’ve included all the necessary attachments with your application to prevent any delays in the processing of your form.
  • Sign and date the form: Your signature is necessary to validate the form. Ensure you also put the current date next to your signature to confirm when the form was filled.

Things you shouldn't do:

  • Leave sections blank: If a section doesn’t apply to you, write “N/A” (not applicable) instead of leaving it blank. This shows you didn't overlook the section by mistake.
  • Use pencil or non-approved ink colors: Stick to the requested ink colors to ensure your form remains permanent and is easily legible under all conditions.
  • Provide false information: Always be truthful. Providing false information can lead to your application being denied or later disciplinary action if discrepancies are discovered.
  • Forget to check for special requirements: Some positions may require additional information, such as a background check or drug testing consent. Make sure you’ve filled out every relevant section.

By following these guidelines, you'll present a complete, accurate, and professional application that will likely be processed efficiently, bringing you one step closer to potential employment with Airsev.

Misconceptions

When looking at the Air Serv Application for Employment form, there are several misconceptions that can easily arise. Clarifying these misconceptions can help applicants understand the process more clearly and improve their application submissions.

  • Misconception 1: Completing the application form guarantees a job offer.

This is not accurate. The application process is the initial step for consideration for employment. It simply indicates your interest and eligibility for the position. Air Serv makes clear that filling out the form does not obligate them to hire an applicant. Hiring decisions are made after thorough evaluation of all applications and interviews.

  • Misconception 2: You only need to provide partial information on the application form.

Every section of the application is crucial. Failure to provide complete information may result in denial of your application. It's important to fill out each section with accurate and detailed information to avoid delays in the review process. If additional space is needed, attaching separate sheets, clearly labeled for each section, is recommended.

  • Misconception 3: Personal information provided on the application is shared publicly.

This concern is understandable but unfounded. The information you provide on this application is kept confidential unless the applicant consents otherwise. Air Serv respects the privacy of its applicants and ensures that personal information is protected, in line with federal and state laws regarding privacy.

  • Misconception 4: Previous legal convictions automatically disqualify you from employment.

While it's required to disclose convictions of felonies or misdemeanors, including DUI (Driving Under the Influence), this does not mean automatic disqualification. Each case is assessed on its own merits, focusing on factors such as the nature of the conviction and how long ago it occurred. Applicants are encouraged to provide a detailed explanation if they have convictions, allowing a fair assessment of their application.

Understanding these aspects of the Air Serv Application for Employment form can greatly assist applicants in accurately completing their submissions and setting realistic expectations toward the hiring process.

Key takeaways

Filling out the Airsev Application For Employment form accurately and completely is vital for job seekers. Here are four key takeaways to ensure the process is smooth:

  • Always use black or dark blue ink and print clearly or type your information. This ensures that your application is legible and professional-looking, which is crucial for making a positive first impression.
  • Do not leave any sections incomplete. If additional space is required, attach separate sheets of paper, clearly marked to indicate the sections they correspond to. Incomplete applications or those pending additional information will not be processed until they are fully completed.
  • Providing false information on the application form is not only unethical but also illegal. It can lead to the rejection of your application or, if discovered after employment, termination. Always be truthful in your responses.
  • The form includes a section for a 10-year employment and background history. It's important to account for any employment gaps of two months or more with verifiable documentation. This thorough background check is a Federal requirement for certain positions, highlighting the importance of accuracy in your employment history.

Remember, the application process is your first step towards employment, so taking the time to fill out the application form carefully is crucial.
